If you are a new patient, you can set up an appointment for an initial assessment to determine if counseling is appropriate for you by calling Behavioral Medicine at 336.547.1574. Autism spectrum evaluations are also available, get details here.

Prepare for Your Appointment
We ask that you arrive at the office 10-15 minutes prior to your appointment time to allow for paperwork, and bring any completed paperwork with you. The office will give you paperwork to fill out, and there will be generic blank paperwork located on a clipboard in the lobby.
Please bring your insurance card so that we may make a copy of it. Our office will contact your insurance company to verify coverage and benefits prior to your appointment. You may contact our office and/or contact your insurance company directly, if you have any questions or concerns. It is your responsibility to understand your insurance benefits. Co-payments are due at the time of your visit and may be paid by cash, check or credit card.
All information is kept confidential in accordance with the Federal Health Information Privacy Protection Act. Our records are separate from those of other divisions within LeBauer HealthCare.
Need to Cancel an Appointment?
We reserve one hour for you, so that it is essential we have sufficient notice (at least 24 hours) of a cancellation. There is a $50 fee for missed appointments, which is not covered by insurance. To reschedule, you may call us at 336.547.1574. You can leave a message and someone will return your call quickly.
If you are not able to reach the office and you are close to the 24-hour time-frame, please call the clinician you are scheduled to see and leave them a voice mail to notify them you will not be able to keep your scheduled appointment. If your appointment is scheduled for a Monday, you will need to call by Friday to notify us of your need to cancel.
