LeBauer HealthCare Elam Avenue Office to Move to New Green Valley Building
Patients of LeBauer Primary Care at Elam and LeBauer Sports Medicine will soon be able to receive care in a new state-of-the-art medical building located at 709 Green Valley Road. The new office, scheduled to open on December 21, features larger exam rooms and waiting areas, a lab, an X-ray area, and an expanded space for sports medicine care, including a concussion clinic. Counseling services will also be offered.
LeBauer HealthCare began construction of the 17,000 square-foot building in early 2020 and plans to move all Elam Avenue services to the new Green Valley office on December 21. The same medical team that serves patients at Elam Avenue will see patients at the new Green Valley office.

- Evan Corey, MD
- Smith has special training in ImPACT, a concussion diagnostic and treatment program, and will also care for patients in the Concussion Clinic.
- Counseling services will be provided by Jessica Thomas, MSW, LCSW, a member of the LeBauer Behavioral Medicine team.

Current primary care and sports medicine patients at the Elam Ave. office have been notified by MyChart and by mail of the upcoming move to the new office. Patients with appointments in December will also be directly contacted with specific directions.
