Ten nurses and medical assistants at various LeBauer Primary Care locations were recognized for these qualities and more at the Cone Health Nursing Excellence Awards held in May. LeBauer is a part of Cone Health Medical Group and the Cone Health Network.

The annual awards are held to recognize exceptional patient care and innovation in nursing practice. This year, there were a total of 60 nurses selected from throughout the Cone Health Network.

“LeBauer HealthCare is honored that ten of our nurses were chosen for these competitive awards,” says Brian Hunt, Director of Primary Care and Behavioral Medicine. “Our nurses are an integral part of our office care teams, working closely with doctors, physician assistants, nurse practitioners, and other office staff to help patients receive the best possible care day in and day out. These ten nurses are exceptional in building patient relationships and creating a culture that continuously enhances the care and services we provide.”

The ten LeBauer nurses and medical assistants who received a Cone Health Nursing Excellence Award include:

Denisa O-Brien, LeBauerDenisa O’Brien-Blaney, LPN, LeBauer Burlington Station

Denisa works as a health coach, helping patients better understand and manage chronic diseases or ongoing health issues. “Denisa brings a wealth of experience and knowledge to her role and has been a cornerstone of our Health Coach program,” says Jason Murphy, Clinical Nurse Manager for LeBauer. “She puts her patients at the center of everything she does.”

Kathy Davis, LPN, LeBauer Burlington Station

Kathy is an expert at building rapport with patients and is also known among her teammates for her positive attitude.

“Kathy has a quiet determination and dedication that drives her to provide exceptional care and service to our patients, often without much fanfare but always with a humble heart,”  says Nancy Falish, Practice Administrator.

Tanya makes it her mission to support patients on their journey to wellness.  “Tanya exemplifies servant leadership and is always willing to help when needed,” says Nancy Falish, Practice Administrator. “She has earned the respect of the providers and staff due to her passion for quality care and exceptional service.”

Jamie Southern-Hizer, LPN, LeBauer Brassfield

Jamie makes sure patients have all the tools and information they need to stay healthy, including how to access any health resources within the community that may be needed. “Jamie exemplifies Cone Health values by caring for patients with a positive attitude, caring for teammates when they need support, and caring for the community,” says Stephen Hunter, MD.

Sheena serves as a mentor and resource for the certified medical assistants in the Brassfield office in order to help improve patient care. “Sheena is always eager to take on new responsibilities and has initiated many new process improvement activities throughout the office,” says  Dustin Austin, Practice Administrator.

“Tamara truly believes in the difference she can make every day as a nurse and a human being,” says  Julie Johnston, Practice Administrator. “She never gives less than 110 percent.” Tamara is dedicated to lifelong learning and providing inspiration for both patients and co-workers.

 

Michelle Jones, RN, LeBauer Horse Pen Creek

“Michelle creates a standard of excellence with the patient at the core of her care, her decisions, and her team,” says Maria Busam, RN. Michelle has helped develop policies and procedures that are invaluable for LeBauer Primary Care. “She is a tireless advocate for patients,” says Busam.

Ashlee is compassionate, approachable, and always willing to go above and beyond for patients and peers, according to Jordan Johnson, Practice Administrator. “Ashlee is an incredible example of servant leadership and patient advocacy,” says Johnson. “Her leadership has inspired positive chance and empowered the team to always put the patient at the center.”

As a health coach, Carolyn works with patients, trains other health coaches, and has worked to create a coaching process that engages patients. She recently earned credentials as a Certified Ambulatory Nurse. Last year, Carolyn elected to spend her vacation helping victims of Hurricane Harvey in Houston.

Shannon works to continuously improve her knowledge and skills in order to provide patients with the best care possible. “Shannon is reliable, skilled, competent, and committed to excellence,” says Erin Alexander, Practice Administrator. “Patients, providers, and staff know they can rely on Shannon for help.”
